On completion of this activity, you should be able to:
Identify which patients with type 2 diabetes might benefit from having their medicines reviewed.
Determine which patients with type 2 diabetes could benefit from having a SGLT2 Inhibitor or a GLP-1A added to their medicine regimen, as per PBS eligibility criteria.
Outline a GP management plan for patients with diabetes using a patient centred and multi-disciplinary approach.
Find up to five of your patients with a diagnosis of type 2 diabetes and CVD or CKD.
For each patient, review their record for their most recent HbAIC, prescribed diabetes medicines, and Medicines Review (see data collection template below).
Actions you could consider from the results of your mini-audit:
If there is a diagnosis of heart failure or CKD, consider prescribing an SGLT2-inhibitor for management of these conditions.
If the patient is not currently at their HbA1C target, review their prescribed medicines and consider escalation of therapy. Consider their comorbidities and the potential benefit of SGLT2-i or GLP1-A.
If there has been no medicines review (MBS items 900, 903, 245 and 249) within the past 12 months, consider referring for an HMR or RMMR.
Note: to be eligible to claim 2.5 hours of Measuring Outcomes you must review a minimum of five patients.
3. Measure the Outcomes
Keep a record of the five patients you identified and document any changes to management that were made (keep this record securely within your practice).
4. Claim your CPD hours
Complete the CPD response form to log your hours.

Review the HMR report in the case scenario. Which of these recommendations (if any) would you incorporate into a medication management plan? What do you consider the highest priority for Ben's medication management? Is anything missing that you think is important?
A certificate confirming your participation and the number of CPD hours awarded will be provided within 30 days.
