Reflect on your own practice compared to the clinical case scenario.
Select up to 5 of your own patients who are taking multiple medicines and may be at risk of medication related harm (note that if you intend to claim measuring outcomes hours you must review a minimum of 5 patients):
Consider if there are any changes you would make to your patients’ management based on the educational materials.
Follow up with your patients as required. Discuss the patient information brochure with them. Determine if they could benefit from a Medicines Review and/or deprescribing.
3. Measure the Outcomes
Keep a record of the five patients you identified and document any changes to management that were made (keep this record securely within your practice).
4. Claim your CPD hours
Complete the CPD response form to log your hours.

Evidence required for claiming Educational Activity hours:
Reflecting upon the educational materials please provide a summary of the issues you would need to discuss with Albert and Josie in the case scenario to implement the tapering plan successfully.
Evidence required for claiming Reviewing Performance hours:
Thinking of the patients you reviewed please provide a summary of how you recognised patients that were at risk from cumulative medicines harm. What did you learn from reviewing these patients? How would or did you determine which of these patients would likely benefit from deprescribing?
Evidence required for claiming Measuring Outcomes hours:
Please outline what data you collected from your patients to determine their risk of cumulative medicines harm and how this compared to best practice in the case scenario? (DO NOT PROVIDE ANY IDENTIFIABLE PATIENT INFORMATION)
Did you identify any patterns of treatment or behaviour from the data you collected that will inform your clinical judgement as it applies to reducing adverse effects from cumulative medicines?
Please provide a summary of the changes you made to the management of those patients?
A certificate confirming your participation and the number of CPD hours awarded will be provided within 30 days.
Participating in this MAIA topic on Cumulative Medicines Risk is voluntary.
