The chart lists the potential cumulative risk of adverse drug reactions (ADRs) associated with medicines and can also be used to see how adjustments to your patients medicines might reduce their cumulative adverse effects. This tool has been developed and adapted using data from the Scottish polypharmacy guidelines 2018, MIMs, the current Australian Medicines Handbook, Credible Meds (https://www.crediblemeds.org/) and other relevant electronic sources.
The list focuses on commonly used drugs and commonly preventable ADRs and is not meant to replace more detailed medicines information sources. The scoring system is binary (either no or yes) with no information regarding relative differences between drugs within the same class and individual patient response.
Where patients are taking combination products each drug in the combination needs to be entered separately unless the individual components are only available in a combination form.
